Summary of Job Duties:
We are seeking a dedicated and compassionate Community Health Worker (CHW) to join our team at the Institute of Community Health Innovation. This role is essential in managing relationships with patients, local community-based organizations, and community members to provide education, resources, and health education. The CHW will coordinate outreach and engagement activities, using a community-informed approach to address disparities in Arkansas. Qualifications:
Minimum Qualifications:
Level I:
High School diploma/GED plus two (2) years' experience as a CHW Must enroll in a certified CHW Training Program within 45 days of employment
Level II:
High School diploma/GED plus four (4) years' experience as a CHW Completed certified CHW training program Mastered all CHW Competencies for Level I
Level III:
High School diploma/GED plus nine (9) years' experience as a CHW Requires CHW certification from ARCHWA Mastered all CHW Competencies for Level 1 & II
