Are you a Licensed Practical Nurse with a passion for improving care quality and patient outcomes? Cowlitz Family Health Center is looking for someone to join as a vital member of CFHC Team—where your clinical skills and eye for detail help shape better systems, safer care, and healthier communities. As an LPN, you'll build strong, trusting relationships with patients to support health maintenance and improve outcomes. You'll use patient rosters to conduct outreach for routine screenings, chronic condition management, and complex care, always with a personalized and empathetic approach. Utilizing motivational interviewing, you'll empower patients to take an active role in their health. Your responsibilities will also include delivering tailored health education, assisting with pre-visit planning, and reconciling medical records to ensure seamless coordination between primary and specialty care. You'll contribute to accurate documentation that supports continuity of care and compliance, while helping drive strategic and operational initiatives aligned with patient-centered medical home (PCMH) principles. You will also participate in cross-training efforts and provide support in other clinic roles as needed, contributing to seamless clinic operations and teamwork, as well as understanding the unique patient needs and barriers to identify opportunities for improving access and engagement in healthcare services.Pay Range:

CERTIFICATION/LICENSURE
Must possess a valid, unrestricted WA state Licensed Practical Nurse license. BLS/CPR certification required upon date of hire.DESIRED EDUCATION, EXPERIENCE & SKILLS
High school diploma plus successful completion of a minimum of one academic year of post-secondary education in a LPN from an accredited educational institution. Associates degree in LPN preferred. 1-4 years previous work experience as a LPN preferred. Proficient in Medical terminology and medical office procedures. Ability to work with a low-income, multi-ethnic patient population and a demonstrated ability to relate respectfully and effectively to people of diverse racial, educational and socio-economic backgrounds. Must be a confident, self-directed individual who can work within a team as well as independently. Strong communication, organization, documentation and record keeping skills necessary.
