29 Hourly Travel Percentage None Job Shift Floater Job Category Nurse Description SUMMARY Under the supervision of the RN Supervisor and/or Program Director, provides support to patients and documentation of self medication.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties and responsibilities may be assigned. On Site daily Support residents in the (self) administration of medication Documentation of (self) administration of medication Performs TB and Drug screenings on all patients within 72 hours of admissions Performs routine urine drug screens as directed by the treatment/recovery plan Monitors and records vital signs Provides triage and provides medical services accordingly For patients with health care needs, schedules and coordinates all medical appointments, and provides periodic follow-up Reviews and transcribes medication orders, and monitors medication records for compliance; Observes and records response to treatment/medication and notifies the Medical Director, the RN Supervisor, and the Program Director of any such instances Monitors ongoing/chronic medical problems Handles minor needs of patients including attending to minor injuries, administering routine medications Maintains all medical records Performs QA activities EKG Performs accurate and complete audits of medication transported, received, administered, stored or disposed, as trained and assigned, in accordance with Phoenix House medication policies and procedures.

REQUIRED EDUCATION
Graduate from an accredited nursing program
LICENSES/CERTIFICATIONS
Current NYS Licensed Practical Nurse Current certification in BLS and/or ACLS REQUIRED
EXPERIENCE
At least three years related experience Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs
WORK ENVIRONMENT
This job operates in an indoor medical clinical environment. This role also routinely comes into contact with patients who may have contagious illnesses. Appropriate gloves required.
