Job Description
- Position Summary
- The purpose of a Community Health Worker (CHW) is to act as the bridge between our patients, community, and medical systems in order to remove barriers and increase wellness across all life domains.
- Candidate must live within driving distance to Jackson Health System in Miami, FL
- As a Community Health Care Worker at Aetna you will:
- + Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to coordinate patient needs + Meet with members in the hospital and/or in their home to conduct a needs assessment + Manage patient referrals defined by the care team & collaborate with the Case Manager on action plan + Knowledge of health plan basics (e.
