Job Description
Summary:
Support team based care initiative within the scope of practice. Provides clinical support to assure smooth daily department operations in assigned medical practice(s). Responsible for completing all visit documentation including Provider exam and proposal of orders. Assures that the patient and examination area is properly prepared. Completes clinical tasks under the direction of the Provider or Registered Nurse.Responsibilities:
1. Follows up with pre-visit planning focusing on care gaps, leading team huddles with the clinical staff. 2. Greets and brings patient back from waiting room to clinical area. Performs all necessary intake activities and documents appropriate information including accurate vital signs, current medications and allergies, verifies EMR reflects current PCP, appropriate referring Provider, preferred pharmacy, etc. 3. Documents and updates patient information in the patient record. Documents demographic, medication history, social and medical history as reported by the patient from the intake forms, or as directed. 4. Completes all office visit documentation with the Provider, including proposing orders for review, as appropriate. Completes tasks with appropriate documentation, Updates medication by history for Provider review. 5. Provides direct patient care, including vaccination and medication administration and basic wound care. 6. Completes appropriate paperwork as needed. 7. Communicates test results as designated by the Provider or Registered Nurse. 8. Assists with prior authorizations and referrals. 9. Propose medication renewals. 10. As delegated, instructs patients and their caregiver on how to perform Provider recommended preventative care, take prescribed medication , prepare for tests and procedures, monitor treatment progress and document symptom changes. 11. Manage tasks and messages in a timely fashion. 12. Schedule future appointments/referrals/procedures/diagnostics. 13. As a delegated agent, obtain state regulated prescription monitoring report. 14. Assures the examination area is clean as per infection disease protocols. 15. Performs environmental rounds as assigned. Complete meaningful use requirements. 16. Assures the patient is properly prepared for the scheduled visit to maintain timely patient flow. This includes assisting patients into gowns, having clinical paperwork, any needed supplies and educational materials immediately available for provider use. Conducts in-office testing per protocol or as directed by the Provider. Coordinates testing and outside ancillary services as applicable. 17. Collects, prepares and processes specimens as directed by the Provider. Completes process by assuring speciment results as logged in the accession logbook and are documented in the patient's record. Assist with Telehealth workflow. Maintains vaccine, medication and supply logs and reports discrepancies to supervisor. Interacts with patients and colleagues in accordance with Western Connecticut Medical Group service excellence standards. 18. Maintains inventory and ensures proper upkeep, cleaning and testing of medical equipment and supplies. Fills out appropriate logs. Participates in supporting the introduction of new colleagues into the office regarding procedures and techniques. Performs audits as assigned by supervisor. Demonstrates and maintains practice-based competencies for in-office surgical procedures if applicable. This includes procedure setup, patient prepping with sterile technique, patient recovery and room turnover under RN direction. 19. Fulfills all compliance responsibilities related to the position. 20. Performs other duties as assigned.Required:
Basic Life Support (BLS) certification within 30 days of starting. Licensed Practical Nurse licensure. Basic medical knowledge and terminology. Computer skills/data entry. Skilled in obtaining accurate patient vitals. Vaccine administration and medication preparation.Preferred:
Minimum Experience:
one year. Proficiency in multiple languages. Phlebotomy. EKG.Education Required:
HS Graduate or Equivalent. Graduate of an accredited LPN Program.Working Conditions:
