Monday•Friday, 8:00 a.m.•4:30 p.m.

Weekend Requirement:

One weekend shift every month

Job Overview:

Join our dedicated home health care team as an LPN, providing compassionate and skilled nursing care to patients in their homes. You will conduct six patient visits per day, collaborating closely with a multidisciplinary team of RNs, PTs, OTs, and HHAs to deliver high-quality care.

Key Responsibilities:

Perform skilled nursing functions in compliance with the State Nurse Practice Act and organizational policies. Deliver care as delegated by the RN Case Manager and/or Clinical Team Manager. Adhere to federal, state, and local regulations, maintaining the highest standards of professional practice. Document patient care activities accurately and promptly. Participate in a weekend rotation schedule, working one weekend shift every month.

Qualifications:

Experience:

Previous home care experience required.

License:

Current LPN license in good standing. Ability to work independently and within a collaborative team environment. Strong organizational and communication skills.
