Job Summary:
Responsible for completion of the MDS/Resident Assessment Instrument (RAI) process. The MDS Coordinator is responsible for coordinating all aspects of the RAI process with the appropriate participation of health care professionals.Qualifications:
Education:
Must be a graduate of an accredited school of nursing; or, a minimum of at least a high school diploma, ability to read, write, follow written and oral instructions, and successfully complete the Louisiana Practical Nurse-training program as required by state and federal regulations and be qualified to instruct nurse aide training program.License/Registration:
Licensed Practical Nurse registered with the Board of Examiners of Practical/Vocational Nurses .Experience:
One year preferred.Job Knowledge:
Must be proficient with computers and American Health Tech, Microsoft Excel, Microsoft Word. Have knowledge of infection control and Material Safety Data Sheets.Responsibilities:
- Enter MDS assessments into American Health Tech and print and transmit MDS data on a timely basis.
- Print Quality Indicator reports monthly or more often if requested by the DON.
- Development and modification of the IPOC; develop and maintain the IPOC schedule; and conduct the IPOC meetings; and ensure completion of the IPOC Summary documentation.
- Ensure completion of the initial plan of care upon admission.
- Completion of the Comprehensive Assessment - MDS and RAP on admission, annually and with significant changes.
- Timely completion of all Medicare MDS utilizing the appropriate Assessment Reference Dates to capture the resident's acuity and level of care.
- Timely completion of the
OMRA 8-10
days after therapy is discontinued if skilled services for clinical reasons are still required. Completion of the Quarterly MDS.- Direct the facility care management system to ensure that the care and treatment planning process is systematic, comprehensive and services are available to meet the resident's medical need to maintain or improve their status.
- Monitor all residents for potential changes in condition.
- Attend and participate in meetings and training sessions including in-service and performance improvement (PI) committee as directed.
- Work overtime, holiday and weekend hours as scheduled and other non-scheduled hours as requested by management.
