Licensed Practical Nurse ( LPN) will implement and coordinate the delivery of care in collaboration with physicians and care associates. Assessment, planning, implementation, and evaluation of our residents Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents' overall condition and behavior. Assist the Director of Wellness in the development and update of resident care plans.
