Position:
LPN/LVN, Dialysis Our nurses function as the hemodialysis team leader and provide day to day direction and supervision to assigned direct patient care staff. Promotes positive public relations with patients, family members, guests, physicians, and coworkers. Uses tactful and appropriate communications in all work-related interactions. Dialysis experience preferred. Applicant must have graduated from an accredited school of nursing. Current state license required.
Qualifications:
Current LPN-LVN license in the state of practice required Current CPR certification required High school diploma or equivalent required Hemodialysis experience at least 6 months required. Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system Minimum of one year's experience in a hospital or clinical setting preferred. Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of Novo Dialysis within 90 days of hire required Novo Dialysis requires teammates to comply with Federal and State requirements related to vaccination against Covid-19. This includes being up to date with vaccinations or having a qualified exemption. For our teammates who work to support in-patient treatments in hospital or healthcare systems, there may be additional booster requirements prior to providing patient care services.
Report to:
Charge RN Duties:
1. Assist with clinical admissions and operational procedures to improve patient health and minimize hospitalizations and missed treatments. 2. Demonstrates effective use of supplies and staff labor hours 3. Perform duties as assigned to meet the patient care and/or operational needs of the facility 4. Complete and document monthly review of patient medication profiles. Update orders and/or administer medications as ordered by the physician 5. May Participate/Lead facility with the implementation of anemia management and medication protocols 6. Provide patient education and follow up as needed 7. Assist with programs target goals for patient outcomes in accordance with quality patient care and company goals 8. Knowledge of and comply with federal, state, local laws and regulations, including healthcare professionals practice act requirements 9. Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations 10. Complete patient admissions process as outlines in policy and procedure, collaborating with team members regarding real or potential problems or needs relative to the treatment provided 11. Complete pre and post treatment physical assessments 12. Initiate and terminate patient treatments to include connecting and disconnecting catheters/access sites and cleaning catheter/access sites per protocol 13. Obtain blood samples for lab analysis as required to complete physician orders 14. May reinforce patient education as needed under the direction of the Charge Nurse and Clinical Facility Administrator 15. Use appropriate safety measures including personal protective equipment (PPE) as necessary 16. Prepare machines for hemodialysis treatments 17. Verify/respond to visual and audio alarms on machines; troubleshoot machine alarms as suitable 18. Remove disposables and clean station after each treatment 19. Be familiar with emergency equipment and all emergency operational measures 20. Obtain pre and post treatment weight of patient per protocol 21. Safely initiate and terminate dialysis treatments per facility protocol, and within scope of practice as allowed by state regulations 22. Monitor patients during dialysis treatments 23. Responsible for accurate documentation of information related to patient treatment 24. Maintain supplies in the Nurses' station and Medication Preparation area. Help maintain supply room clean and orderly 25. Perform proper procedure of water testing at the beginning of each shift by the CHT's and as directed by the Charge Nurse. Complete/document quality control logs throughout the day as required. 26. Take vital signs per protocol during patient treatment and document on treatment record as required. 27. Perform safety checks for each patient by reviewing machine settings and treatment settings for every treatment. Review each patient flowsheet for content and accuracy prior to signing it off. 28. Report any significant information or change in patient condition to the Charge Nurse 30. Report machine problems to the Biomedical Technician and Facility Administrator 31. Participate in staff meetings as required. Attend in-services and continuing education sessions in compliance with company policy and procedure. 32. Assist with staff training as requested. 33. Participate in team concepts and promote a team effort, perform duties in accordance with company policies and procedures 34. Participate/Lead facility initiatives (ie: Vascular Access, Care Plans, Transplant engagement, etc.) in accordance with company policies and procedures 35. Must be flexible with the schedule. Avail to work Saturdays as needed 36. Administer medications as assigned in accordance with company protocols, and policies and procedures
